Admission Information
The following items are required for each resident admitted to the New Hampton Care Center:
● Selection of attending physician and physician order for admission, and treatment ● History & physical completed by physician within 30 days prior to admission OR within 48 hours after admission (Form provided by facility) ● Selection of pharmacist ● Copy of Social Security Number ● Copy of Medicare card ● Copy of private/co-insurance ID card including policy number, Insurance company, and address Private Rooms with Private Baths and Carpeting are available ● Name of mortuary ● Medicaid application OR Medicaid number (If applicable) as we are certified for Medicaid ● Copy of legal document indicating Power of Attorney, OR Conservator, OR Guardianship (If applicable) ● Copy of any/all Advance Directives ● Payment upon admission for first month ● All clothing and personal items must be marked with the resident's name.
It is the policy of New Hampton Care Center to admit and treat all residents without regard to race, color, national origin, disability, communicable disease, religion, or age. The same requirements for admission are applied to all.
Administration Total Quality Management
Our Administrator is responsible for assuring that all state and federal regulatory issues and company expectations are incorporated into New Hampton Care Center's policies and procedures.
Very importantly, the Administrator is responsible for making sure the residents' needs are met through each departmental function.
The Administrator also coordinates admissions, policies, procedures, staff relations, marketing and community and customer service.
Social Services Leading the Way
Moving into a care center can be a traumatic experience not only for the resident, but also for the family. Our Social Service Department plays a major role by leading the way in providing assistance, answering questions, and finding appropriate resources to help the resident adjust to life at New Hampton Care Center.
The primary focus of this department is to assure that the psycho-social needs of the residents are met, which include: adjustment, behavior, financial concerns, resident/staff relations, family relations, ensuring appropriate placement, and identifying personal needs of the resident. The Social Service Directors also communicates and ensures residential rights.
Medicare Additional Financial Coverage
New Hampton Care Center is a skilled Medicare Certified Facility. There are two parts of Medicare, Part A and B.
Part A Medicare skilled is a in-patient status where the level of care is skilled and a resident who has met the basic qualification to get Medicare A benefits is a 3-Midnight stay in an acute hospital under acute care. Then there are 20 days that are paid for Medicare in total and then on the 21st Day the Co-Insurance or supplement to Medicare picks up the first $97.00/Day and then Medicare pays the Difference. The level of care is based on the Ruggs III classifications of level of skilled care. This is how the skilled facility is reimbursed for services. Each resident is assessed on an individual basis. Medicare A covers room and board changes, medications, OT, PS, ST Rehab Services, medical supplies, treatments and nursing care.
Part B is an out-patient status that resident may receive OT, ST and PT services and be covered under Medicare part B at 80% of actual fees for services and 20% from supplement or co-pay to Medicare.
