Individualized care plans
Every client receives a personalized care plan developed by a Registered Nurse, tailored to their unique medical needs, daily routines, and personal preferences.
RN-led, locally owned in Concord — delivering personal care, companionship, respite, and skilled nursing across New Hampshire. As your needs change, our care changes with you.
A licensed Registered Nurse visits your home, listens to what your family needs, and builds a personalized care plan. We answer your questions, walk you through every payment option (CFI Waiver, private pay, LTCi, VA benefits), and never pressure you to commit.
Most agencies are built around a single leader — usually clinical, sometimes business. NH Care Center is built around a team. An experienced Registered Nurse leads every plan of care. A systems engineer built our operational and IT infrastructure from the ground up. Two team members bring hands-on home care experience from the field. And one brings years of running a service business where every customer interaction matters. That breadth isn't accidental. Clinical leadership ensures every visit, every assessment, every change in condition is RN-supervised. Operational leadership ensures the agency runs reliably — schedules don't fall apart, your information stays private, and we can grow with your family's needs without losing the personal attention you started with. The home care experience on our team means we understand what actually works in the field, not just on paper. And the service-industry mindset shapes how we treat every family we serve, every day.

Your loved one's health information lives on infrastructure built specifically for healthcare data — the same security standards used by hospitals and large medical groups. We didn't outsource this to a third-party platform. We built it in-house, maintain it in-house, and it's audited continuously. For families, that means your personal and medical information stays protected. For referring clinicians, it means zero PHI friction at discharge.
With every vendor that could touch your information — required by HIPAA, audited annually.
Every staff device is encrypted, monitored, and centrally controlled — no personal phones or laptops in the workflow.
Carrier-level authentication (SPF/DKIM/DMARC) on every message — your information is verified, not spoofable.
Every record access is logged and traceable. We can show exactly who saw what, when, and why.
